APPLY FOR MEMBERSHIP INTO THE

MILITARY ORDER OF THE PURPLE HEART

To apply for membership into the Military Order of the Purple Heart please comply with the

following requirements:

1. Complete the application below.

2. Along with the application you must provide evidence of the award of the Purple Heart.

3. Be advised that a certificate alone does not constitute proof of the award.

4. If discharged, a discharge document such as DD-214 must be provided reflecting

character of service as honorable or under honorable conditions. The following

documents may be submitted as proof of receipt of the Award:

• DD214

• DD215

• WD AGO 53-55

• Copy of your orders or a

• Letter of Transmittal

5. If on active duty a copy of your orders or a letter of transmittal is sufficient.

6. The Lifetime Membership fee is $200. Payment options are noted on the form below. Do

not send cash!

7. You may submit this application on line directly to MOPH National HQS by following

this link www.purpleheart.org . If also submitting a request for Connecticut Purple Heart

License Plates complete and print the membership form below and mail all required

documentation along with your license plate application to the following address:

 

Please note that the license plate application will be held until approval of the

membership application.
